P45, P60 and P11D: What Each Form Is For
A P45 shows the tax you have paid so far in the tax year when you stop working for an employer: give parts 2 and 3 to your new employer. A P60 is your end-of-year summary, which your employer must give you by 31 May if you work for them on 5 April. A P11D shows benefits such as a company car. You cannot get a replacement P45, but you can ask for a replacement P60.

The Forms
P45
Shows tax paid on your pay so far in the tax year when you leave. Give parts 2 and 3 to your new employer, or to Jobcentre Plus if you are not working. Keep part 1A. You cannot get a replacement; your new employer can use a starter checklist. source
P60
Shows the tax paid in the tax year, one for each job. If you are working for an employer on 5 April, they must give you one by 31 May. You need it to claim back overpaid tax or prove your income. Ask your employer for a replacement if you lose it. source
P11D
Shows benefits such as a company car. Your employer must tell you how much each benefit is worth; you might not get one if tax on the benefits is taken through your pay. source
